Insurance Coverage

Neuropsychology Assessment Service does not accept direct insurance payment and is not a participating provider in any insurance network. Our services may be covered by your insurance carrier as an out-of-network provider. It is your responsibility to make any necessary arrangements with your insurance company regarding reimbursement of services provided. A billing statement will be provided to you that you may submit to your insurance carrier.

Cancellation Policy

Psychotherapy sessions not cancelled 24 hour prior to the appointment will incur a $100 missed appointment fee regardless of the reason for cancellation.

Testing Appointments not cancelled 5 days prior to the scheduled appointment will incur a $350 cancellation fee regardless of the reason for cancellation.

Payment

Payment is expected in cash or by check at the time of service with the exception of forensic evaluations, where other arrangements  are made. Make all checks payable to: Dr. Robert Kraft. There will be a $30 fee for returned checks. 

Clinic Fees

Hourly rate (Non Forensic):

$ 150- 60 minutes

$110- 45 minutes

Neuropsychological Test Battery (Non-Forensic):

Initial Consult (90 minutes) - $225

Testing, Scoring, Report Writing - $1050

Feedback Session (90 minutes) - $225

Total Cost - $1500

Neuropsychological Test Battery (Forensic):

Minimum Rate -$ 2000- up to 10  hours; $200 per hour thereafter.

Psychological Evaluation (Forensic)

Minimum rate - $1000- Covers up to 5 hours; $200 per hour thereafter.

Child Custody Evaluation - $3000

Deposition & Expert Testimony

$200 per hour with 2 hour minimum
